Check your plan for coverage requirements and out-of-pocket expense for pre-surgical, surgery and post surgical requirements. We recommend you review your insurance policy to see if you have:

• Weight loss surgery coverage
• Any out-of-pocket expenses
• Additional requirements you must meet for coverage

Insurance companies usually require at least six to twelve consecutive months of documented diet history. Many also require nutrition and psychiatric consultations. WINH’s pre-surgical Lifestyle Program will help you meet insurance requirements.
